Top Weight Loss Medications

Clinical research, outcomes in the real world, and what patients are actually asking for all point toward the same short list. These are the strongest candidates for the best weight loss medication today:

GLP-1 and GIP Agonists

Wegovy (semaglutide)

  • The gold standard right now is still Wegovy (semaglutide), which has turned in consistent clinical results of 10–15% body weight reduction. It carries FDA approval and is prescribed widely for patients with a BMI ≥30, or ≥27 alongside weight-related conditions.

Zepbound (tirzepatide)

  • Zepbound was the first dual GIP and GLP-1 receptor agonist to win approval for weight loss, and trials have shown up to 22% weight loss. Results may arrive faster, though gastrointestinal side effects can run heavier.

Ozempic (semaglutide, off-label)

  • Ozempic holds its FDA approval for type 2 diabetes, yet it is prescribed off-label for weight loss all the time, usually where Wegovy is unavailable or insurance will not cover it.

Mounjaro (tirzepatide, off-label)

  • Until Zepbound won approval, Mounjaro was the standard off-label tirzepatide prescription for weight loss. Plenty of patients stay on it successfully, particularly where an insurance plan covers it.

Non-GLP Medications

Contrave (naltrexone/bupropion)

  • This combination acts on the reward system in the brain, cutting food cravings and emotional eating. It tends to go to patients who cannot tolerate injectables or who have contraindications for GLP-1 therapy.

Qsymia (phentermine/topiramate)

  • Qsymia pairs a stimulant with a drug used for migraines and epilepsy. It is FDA-approved for weight loss and may fit patients who want an oral medication that supports appetite suppression along with increased energy expenditure.

Orlistat (Alli, Xenical)

  • Orlistat inhibits fat absorption, blocking about 25% of dietary fat from being taken up, and it is sold over the counter as well as by prescription. The price is easy on a budget, but the weight loss is usually less dramatic (~5%).

Compounded GLP-1s

Compounded semaglutide or tirzepatide

  • Compounding pharmacies prepare these versions, and a provider may reach for them when the branded medications are not accessible or not affordable. They are not FDA-approved, though they may be beneficial where they are sourced and supervised properly.

New and Emerging Medications

In Development

Currently, a number of additional injectable and oral candidates sit in late-stage clinical trials, among them once-monthly formulations and GLP-1 hybrids built for enhanced tolerability. What this next generation of compounds aims at is less nausea and better metabolic markers.

What to Watch For

Watch the press releases and updates coming out of the FDA, Novo Nordisk, Eli Lilly, and the other major pharmaceutical companies, because new treatments keep widening the range of options open to patients and providers alike.

How Do These Medications Actually Work?

GLP-1 & GIP Receptor Agonists

These imitate the natural hormones behind hunger and insulin response. Gastric emptying slows, cravings drop off, and insulin sensitivity improves.

Appetite Suppressants

Phentermine, Contrave and similar stimulant-based or neuroactive medications act centrally, turning down food-related impulses and overeating behavior.

Fat Absorption Inhibitors

Orlistat stops the body absorbing dietary fat. Appetite is left untouched, but paired with a low-fat diet it can be effective.

What Are the Side Effects of Weight Loss Medications?

Common Across Most Medications

  • Nausea
  • Constipation or diarrhea
  • Headaches or fatigue (especially early on)

Medication-Specific Reactions

Vomiting or bloating may turn up with GLP-1s while the dose is being titrated. Anxiety or dry mouth can come with Contrave. Qsymia may raise heart rate or bring on insomnia. Orlistat has a reputation for GI issues when dietary fat intake runs high.

Who Should Not Take These Medications?

Medical Contraindications

A history of pancreatitis, thyroid cancer, certain mood disorders, or cardiovascular disease may rule a person out as a candidate for some of these medications. Check with your provider before you begin any new regimen.

Medication Interactions

Absorption of oral contraceptives may be altered by GLP-1s. Contrave may not suit someone taking SSRIs. Every medication you currently take belongs in the conversation with your provider.

Best Weight Loss Medication by Category

No single medication is genuinely the “best” for everyone. What is ideal for you turns on your goals, your health profile, how well you tolerate side effects, and your insurance. Appetite suppression suits some patients better, while others need metabolic support or help regulating blood sugar.  The rundown below shows how several of these medications stack up, though VDPC in Portland always recommends working with a licensed medical professional to determine what is right for you.

  • Best Overall: Wegovy or Zepbound
  • Best for Insurance Access: Ozempic or Mounjaro
  • Best Budget Option: Orlistat or compounded semaglutide
  • Best for Appetite Cravings: Contrave
  • Best GLP-1 Alternative: Qsymia
